We accommodate young people that are making the transition from residential or foster care to semi-independence. We consider all young people from different backgrounds regardless of their history or circumstances. A comprehensive referral, needs and risk assessment is completed for every young person referred to the service as per ‘Other Arrangements’ under the Care Planning, Placement and Case Review Guidance – Volume 2, the Children Act 1989.

We consider emergency placements at the service; however, the same admissions procedure is followed. Where practical, we aim to ensure that every young person views the accommodation/placement prior to moving in. We ensure that their wishes and feelings are taken into account and their education, training and employment need to be considered when assessing the suitability and location of the placement/accommodation.

We would not consider referrals for young people where it is identified their needs could not be met effectively. Compatibility for young people who are sharing is an important consideration when reviewing referrals.

  1. It is important to have full information on the young person’s needs in order that the matching process is as reliable as possible from the referral stage.
  2. Upon agreement being reached regarding the suitability of the proposed placement, a pre-placement planning meeting should be held whenever possible.
  3. Prior to the placement, Hopefor16Plus and the local authority will discuss:
  • The purpose of the placement and desired outcomes
  • Duration of the placement
  • The services beyond the placement which will be required
  • Contact arrangements.
